For each landline – connect the leftmost port to any wall jack using a standard phone cord; connect to a power outlet using a Class 2 transformer power adapter (12v 500mA). No need to plug a phone into the middle port (but you can if you want to); this will work for all phones on that line. If you have a 2nd line, you’ll need a 2nd TeleZapper.
Phone with answering machine – using standard phone cords, connect the leftmost port to any wall jack, connect the middle port to the answering machine, connect the answering machine to your phone. You’ll need to re-record your greeting and incorporate a 2-second delay at the beginning to allow the TeleZapper to do its thing. Connect to power as in #1.
Phone with callerID device – using standard phone cords, connect the leftmost port to any wall jack, connect the middle port to the callerID device, connect the answering machine to your phone. Connect to power as in #1.
Phone with both answering machine and callerID – using standard phone cords, connect the leftmost port to any wall jack, connect the middle port to the callerID device, connect the callerID device to the answering machine, connect the answering machine to your phone. Connect to power as in #1. Re-record greeting as described in #2.
I don’t have experience with the TeleZapper TZ900, but its likely the setup is similar.

Oops! – on #3 replace ‘answering machine’ with ‘callerID device’. My bad.
